I'm not sure where you are from but in my area, the insurance companies count on you not fighting them. I have had numerous incidences where they refused to pay for other treatments. I write a few nasty letter and leave a big paper trail.
The standard joke here from my husband (the cluster sufferer) when we get a bunch of checks from the insurance is... Have you been writing letters again?! Write them to everyone. State your position. Any ??? give me a shout, I'll help if I can.
