My insurance company restricts Imitrex the same way. I use the 20mg nasal spray and they will only pay for 6 doses every 34 days. It is strictly a financial thing and has nothing to do with safety.

Reading from the instructions on my current box of Imitrex nasal spray it says, "If the headache returns, the dose may be repeated once after 2 hours, not to exceed a total daily dose of 40 mg." Of course all the information relates to migraine and not a word about clusters, even though I can't see what the difference would be.

Just my opinion, but if I had to inform my doctor of the types of medications available to treat clusters I may find another doctor. I must confess that I've done that before, many times but I finally wised up. There I was teaching a doctor something that he should already know and he is charging me. There's something wrong with that picture.
