8 Years, Worst Yet, Help!

After eight years, this episode is by far the worst. Usually, it's every 18 months (near Spring or Winter solstice,) lasts 4-6 weeks, two-a-day at 60 min each. Pretty classic.
 
This time, however... 5 days in, off-the-scale pain radiating from jaw, as oppsed to radiating from the temple TO the jaw, as is usual (for me.) Attacks last hours - 3 on the short to 12 on the long. Again, very unusual. Did I mention how unbelievable the pain level is? Added bonus - swollen, tender gland. What's that about? That's new and improved - which brings me to the first of a couple of questions...
 
Is this gland thing to be expected? Or is it possible I got some sort of mouth infection coincidentally? I've read posts from others who have or are ready to pull their teeth - with me, I'm ready to yank my entire jaw out. There are no signs of an infection on the outside - no redness, etc - yet the gland is very swelled and super tender. And the fact that the pain is radiating from the jaw has me wondering...
 
If I call 911 after, say, 3-4 hours of relentless, soul-wrenching pain, will they show up with relief? Will they insist I go to a hospital? Will a hospital ER offer relief?

Regarding the swollen gland - I'm not a Dr., by any means, but unless you would have a known reason for the gland to be swollen, ie., upper respiratory infection, etc., it should be examined to determine why it's swollen and/or sore whether in cluster cycle or not. Possible infection, etc. Not trying to alarm you, I've always been told that when your glands become sore or swollen, your body's usually trying to fight something off.

Frank I would definately go to the doc, somethin ain't right there.
