Phil, This is what we did..I thought it might help you.
our Doc wrote a letter to our ins. He included all meds that Rich had previous taken
and had not worked. He stated that in his opinion
It was medically nessasery for Rich to use Imetrix.
We were approved for 10 boxes a month..maybe this will help you.
