Economics -- Perhaps there is some bookkeeping error in which the cost of the pain is charged to some other account.
Electronics -- Perhaps there is an electrical-diode which has developed which prevents the feedback loop from operating as designed.
Perhaps the euphoria-seeking part just doesn't care about the pain. Perhaps it doesn't know about the pain.
Sorry if the analogy confused.
