Was on 960 for better part of two years and know of others who've been there for longer. Have even seen in the literature (and no, I can't remember where, that 1200 has been used). (480 is a high dose maybe for veraps non-cluster uses.)
As to whether it's working or not, many have the experience that by gradually upping the dosage you knock the beast back down for a while, but may have to increase again, as he decides he can cope with it!
The heart problem potential is easily solve by having ECGs (US - EKGs?) at each move up in dosage, and regularly thereafter.
