Would I guess right you work for a large or successful firm?
They in turn would then most likely have an agency that would handle disabilty claims seperately from the company. Which is easier on you. You would be assigned a case worker, and not have to constantly deal with your employers.
Who knows, may even get better than 60%? You'd also get to choose how much, if any taxes you wanted taken out. Of course, it'd come up at the end of the year anyway.
Paperwork for CH is not very difficult at all. The toughest question is "expected date of return?" Now how you supposed to answer that one?!
(It can be changed tho);) You and your doctor will both fill out forms, takes about 2 weeks, but you should get back pay.
