The reason you (and others) still get the occasional breakthrough headache, and shadow-like phantom CH symptoms even after psilocybin therapy is that the serotonin-regulating mechanism has not yet been completely reset.

Don't think of it as an "all-or-nothing" procedure such as flipping a switch. Think of it as closing a water faucet. In your case, the faucet has not yet been completely closed.

Those who have had the very best success with this treatment are those who take the psilocybin either as a pre-emptive strike, between cycles (like someone who walks by a closed faucet every now and then and gives it a twist just to make sure it is shut, even if it is not leaking at the time) or those who take the psilocybin right at the very first warning signs of an impending cycle (like someone giving a twist to a faucet that starts to drip in the middle of the night).

Those who are trying to end a cycle that is in "full flood", where the faucet has been spun wide open, need more than a twist to shut that faucet. Even after several twists, there may still be a bit of leakage, because the sealing gasket has been shifted out of position by the raging flow of the water that has gone on for weeks or months. Eventually, through repeated "twists" or through a gradual natural shift, that gasket nudges back over the final millimeter or two and the faucet can be completely shut once again.
