Mine differ depending on when I get them. The ones that wake me from a deep sleep are as you describe, completely debilitating.

Fortunately, most of my attacks are not that extreme, and I've never suffered an extreme attack that I didn't wake up with. Though twice I did have an attack that I developed during the day that put me on the floor writhing in pain, but even this was less than the full blown attack, as I could still understand what my wife was saying.

My attack today was more typical though, it hurt worse than pretty much any other pain, but was still way down from the most extreme.

It's hard to explain, one can be as painful as having your head cut off x 2, but the other is worse than having your head cut off x 1000.
