Pain is a very subjective "thing." When we have determined that 0 = no pain at all and 10 is the worst that it can get, then there is no such thing as a 10+. A pain can be worse then previous headaches, but that just means that said person has to assign a new value to 10.
Since what I feel as a 10 may not be what you feel as a 10, that doesn't mean that I feel a 9 or an 11. It means that my heaviest level is what my subjective self has told me what it was.
Now, another point is, if a 10 is where we can become suicidal, a 10+ would be where we committed suicide. I'm pretty certain that I can't post during a 10. And I'm even more certain that I wouldn't be able to post after offing myself.
