Was your husband definitely diagnosed with clusters? The cycles of clusters have never really been understood. There was a recent discussion (page back about two days)about the effect of surgery on CH cycles. Various folks have had different experiences. It is within the realm of possibility that surgery could break the cycle.
The very terms "chronic" and "episodic" are vague, and do not portray the true experience. I once was episodic, but now am chronic. I have the same pain, but more frequently than before, and the pain rarely goes away, just ebbs and flows up and down the scale.
Have him see a good neuro, get a good diagnosis, and get some good meds, especially abortives. Oxygen is the treatment of choice for relief during an attack, because it (usually)has no side effects. Good luck to both of you.
